The aim of good incident investigation is to establish the sequence of events that ACTUALLY took place and resulted in the damage or loss so that you can compare this with the sequence of events that SHOULD have taken place

• Objectively and collectively identify all contributing causes.

• Collect information.

• Analyses data to identify contributory factors.

• Determine the root/real cause of events.

• Design appropriate action plan to prevent further losses.

• Create ownership amongst employees.
